Post-Session/Activity Required Information

This "Post-Session/Activity Required Information" form must be completed after the session, once all attendance has been entered and any required documents are uploaded.

After all necessary documentation has been completed and uploaded into the Session, please complete the "Post-Session/Activity Checklist" form in your assignments in the CME database.

When this form is submitted, it will automatically notify the CME office indicating everything is complete and ready for review. The CME office will review the documents and issue an invoice to you for the application and credit fees. Upon receipt of payment of the invoice, the CME office will email the participants with instructions on how to log into the system to complete the evaluation and print their certificate.

To complete the form you will need the following information:

  • Total attendance. This is the total number of attendees, not just the ones who you are requesting credit for. It will be broken out in the following categories:
    • Physicians
    • Nurses
    • Pharmacists
    • Physician Associates (formerly Physician Assistants)
    • Optometrists
    • Social Workers
    • Other
  • Details on any funds received from outside sources
  • Confirmation that all ther documents have or will be uploaded
